Title:Urinary excretion of renal brush-border enzymes in lepromatous leprosy - a preliminary investigation.

Abstract:Activities of the brush-border enzymes, alkaline phosphatase, maltase, leucine aminopeptidase, and gamma-glutamyl transpeptidase, were measured in urine samples of 25 lepromatous leprosy patients and an equal number of age-matched healthy controls. None of the patients were shown to be suffering from any other systematic disease. The enzymatic activities were shown to be significantly elevated in leprosy patients when compared to controls^ien.

Title:Hepatic morphology in reactional states of leprosy.

Abstract:Liver function tests and liver biopsies were studied in 23 leprosy patients in reaction and 10 without reaction. The liver biopsies in leprosy patients with reaction showed exudative lesions, epithelioid and tuberculoid granulomas, and foam-cell granulomas. Portal vasculitis was encountered in a few cases. Neutrophilic infiltration into the foam-cell granulomas was seen in a few cases of lepromatous (LL) leprosy with reaction. In six cases of borderline (BL, BB and BT) leprosy with reaction, a spectrum of lesions bearing footprints of exudative lesions were seen evolving into epithelioid-cell granulomas. Foam-cell granulomas and tuberculoid and epithelioid granulomas along with exudative lesions were encountered in two cases on individual biopsy strips. An altered albumin-to-globulin ratio was the chief functional derangement observed in these cases. The spectrum of changes observed in borderline leprosy with reaction could be discrete steps in the evolution of upgrading reaction^ien.

Title:Nocardia farcinica pleuritis in a lepromatous patient with severe necrotizing reaction: an unusual presentation.

Abstract:A young, male, lepromatous leprosy patient with a severe necrotizing erythema nodosum leprosum reaction treated with prolonged oral steroids and thalidomide developed pleuritis that was caused by a rare opportunistic pathogen, Nocardia farcinica. This organism was resistant to most antibiotics but was susceptible to amikacin and minocycline. During the course of treatment the patient developed severe gastritis which necessitated the removal of clofazimine and the inclusion of an H2 receptor antagonist. Bilateral steroid-induced cataracts needed surgical correction. This case is being presented for its rare opportunistic bacterial infection and for the multiple complications which made treatment difficult. (AU)^ien.

Abstract:Estudam caso de hansen¡ase tuberculóide da variedade nodular da infância (Souza Campos), observada em criança de 2 anos e 7 meses, do sexo feminino, branca e que desde o nascimento mantinha contato com o pai, portador da moléstia do tipo V, com baciloscopia fortemente positiva (+++) em L.C. e M.N. Apresentava 4 nódulos localizados no membro superior esquerdo, sendo o maior de 11 X 10 mm e os demais de 4 X 5 mm. Eram duros, indolores, aderentes aos planos adjacentes, de superf¡cie lisa, de tonalidade eritematosa e, quando distendidos, tornavam-se translúcidos e de cor branco-amarelada. O exame histopatológico revelou granuloma tuberculóide (tipo sarcóide) com ausência de b.a.a.r., os quais também foram negativos em baciloscopia de L.C. e M.N.. A reação de Mitsudina foi fortemente positiva (nódulo ulcerado de 8 X 11 mm). Na evolução, as lesões mantiveram-se estacionadas nos primeiros 17 dias; do 35º ao 70º dia houve aumento de tamanho e a partir do 87º dia iniciou-se a regressão. No 125º dia de observação a redução era de 50%. Discutem a influência do fator idade na incidência da moléstia e apresentam os argumentos que justificam a individualização dessa variedade de hanseníase. (AU)^ipt.

Abstract:A pilot study has been undertaken to compare the efficacy of small dose pulsed betamethasone therapy with need based oral steroids in chronic recurrent erythema nodosum leprosum (ENL) patients. Though this mode of therapy was well tolerated, no advantage with intermettnt steroid administration was observed. This coul have been on account of small dose of steroid given monthly. Treatment of chronic recurrent erythema nodosum leprosum (ENL) patients continues to be unsatisfactory, particularly, because of non-availability of thalidomide. Though corticosteroids are effective in suppressing all the manifestations and even restoring partially or fully the functional impairment, their side effects and dependence are equally troublesome. Based on (a) the reported efficacy and safety of intermittent use of corticosteroids in several immune complex mediated disorders (Cathcart et al 1976, Kimberly et al 1979), Lieblin et al 1981 and Pasricah & Gupta 1984) and (b) ENL (type II) reactions having similar pathology, a pilot study has been undertaken to see the efficacy and the tolerance of pulsed steroids in chronic ENL patients (AU).

Abstract:(Role of temperature, microtraumatisms by elongation or subluxation and canalar stricture.) Cold has a slight aggravating effect on leprosy which is observed in some countries as Iran, South Russia, North India and the Andean cordillera. The subluxation of the ulnar nerve has facilitating effect in the development of the neuritic damage in a limited number of patients. The elongation is important for the production of the ulnar neuritis. The canalar stricture is a major factor causing nerve damage in leprosy. It explains the segmentary localization of the principal damages of the leprous nerves in the proximal areas situated above the tunnels. Intraneural hypertension appears first and is responsible for the swelling of the nerve which gets entraped inside the tunnel. A circulus viciosus is, then, created which maintains and aggravates the intraneural hypertension.

Abstract:A brief review of the contradictory opinions of different authors as to the nature and the place of tuberculoid cases of leprosy in the evolution and pathogenesis of the disease is presented. Mention is also made of their failure to agree on what could be generally considered as the esential histological features of this condition. A case of tuberculoid leprosy which has undergone transformation into a lepromatous-type case within six years of observation is presented. This case report together with the clinical, bacteriological and histological studies of other authors seem to agree with the opinion of Manalang that tuberculoid leprosy is a stage in the evolution of leprosy, not immutable as assert by Schujman. That such transformation among adult tuberculoid cases is rare because they represent the filtered resistant ones or benign infections, is indicated. The need for continued follow-up and life-long study of contact children born of leper parents in institutions is indispensable if we are to know the evolution of leprosy. (AU).

Abstract:Rheumatic manifestations are a common feature of leprosy. The present study was performed to assess the incidence of rheumatic manifestations in leprosy. Methods: seventy cases of leprosy attending the indoor and outdoor departments were studied. Results: Out of the 70 cases studied, rheumatic manifestations were seen in 61.42% of cases: arthritis in 54.28% and soft tissue rheumatism in 17.14%. Enthesitis was seen in 2.84% of cases. The incidence of arthritis in lepra reactions was 57.57%. Conclusions: Musculoskeletal manifestations can occur at any time during the infection. Articular inflammation in leprosy usually occurs in reactive states, particularly erythema nodosum leprosum (ENL). Leprosy can present with arthritis as the first symptom.

Abstract:Some of the possible factors involved in the genesis of amyloidosis during the course of lepromatous leprosy have been studied. In a group of 101 patients from the U. S. Public Health Service Hospital at Carville, La., it is estimated that 40 to 50% have concomitant amyloidosis. This high proportion has been previously wll-documented by autopsy studies (2,3) and was suggested by a 31% positive result in 69 gingival biopsies among this group. Congo red results were often positive, particularly when accompanied by persistent albuminuria. The high percentage of amyloidosis at Carville lies in distict contrast to that of 119 patients studied at Guadalajara, Mexico, where amyloid was diagnosed in only 6%. Factors which may play some role in this different include discrepancies in animal fat consumption between the two population groups. Patients at Carville eat fewer calories but twice the percentage of saturated fat in their diet as compared to the Mexican group studied...AU.

Abstract:The reactional states seen in lepromatous form (erythema nodosum, leprosum, exacerbation) and i interpolar forms (tuberculoid exacerbation and tuberculoid reversal reaction) result in cutaneos, nervous, lymphatic, visceral and polyarthritic acute manifestations. The sucessif relapses result in articular, cutaneous and muscular sequelae of the hand but a paralysis of medial and ulnar nerves, often associated with leprosy, make them different from rheumatoid arthritis. They can be summarized thus: 1- Fixed claws hand associated with stiffness, erosions and sometimes subluxations of interphalangeal joints, mainly proximal and metacarpophalangeal joints...AU.

Abstract:Montenegros reaction was found negative in all cases of cutaneous tuberculosis with ganglionar involvement observed. Our statements differs with those from authors that have studied formeerly the same subject.
